Abstract
A melatonina é um hormônio central na regulação do ritmo circadiano, essencial para a homeostase energética, resposta imune e controle da inflamação. Sua síntese endógena depende de precursores e cofatores como triptofano, vitaminas do complexo B, magnésio, zinco e ômega-3, podendo também ser obtida pela dieta. Este trabalho avaliou as associações entre a ingestão de melatonina alimentar, a adequação de nutrientes envolvidos na síntese endógena de melatonina (escore MRNAS) e desfechos de saúde em adultos do Estudo CUME+ (2016–2024). A ingestão alimentar foi avaliada por meio de um questionário de frequência alimentar validado (144 itens), utilizando uma base inédita de melatonina para 119 alimentos. Os desfechos incluíram obesidade (Índice de Massa Corporal (IMC) 30 kg/m²), depressão (diagnóstico médico autorrelatado), ambos previamente validados para esta coorte, duração de sono (autorreferida), além de condições cardiometabólicas (hipertensão, diabetes mellitus tipo 2, dislipidemia, síndrome metabólica e apneia obstrutiva do sono), obtidas por meio de diagnósticos autorrelatados e dados clínicos/bioquímicos. Foram utilizados como covariáveis: sexo, idade, estado civil, renda, atividade física, duração do sono, tempo de tela, uso de medicamentos para controle de peso, tabagismo, consumo de álcool, de cafeína e de ultraprocessados, além do IMC basal. As análises estatísticas foram conduzidas no software Stata (versão 19), adotando-se nível de significância de 5%. As análises descritivas incluíram testes de média e de frequência (Kruskal-Wallis, Mann-Whitney e qui- quadrado). As associações envolveram modelos de regressão logística e de Poisson com variância robusta, para prevalência, além de modelos de riscos proporcional de Cox para incidência, ajustados por potenciais confundidores. Como resultado, o consumo diário de café, feijões/lentilhas e arroz foi o principal contribuinte para a ingestão de melatonina alimentar (média de 25.554,7 ng/dia). Na análise transversal (n=8.320), observou-se que a ingestão intermediária de melatonina esteve associada a menor prevalência de obesidade (14.965,4–34.426,5 ng/dia), correspondendo aos quintis 2 (RP=0,81; IC95%: 0,69–0,96), 3 (RP=0,72; IC95%: 0,60–0,86) e 4 (RP=0,79; IC95%: 0,62–0,94). Também foram observadas associações inversas com depressão para a ingestão intermediária (20.567,7–34.426,5ng/dia), correspondendo aos quintis 3 (RP=0,79; IC95%: 0,67–0,93) e 4 (RP=0,80; IC95%: 0,67–0,94), sugerindo uma relação inversa e possivelmente não linear. No seguimento longitudinal (média de 4,4 anos; n = 3.736; incidência de 17,9/1.000 pessoas-ano), o quintil 3 apresentou menor risco de obesidade (HR 0.57; IC95% 0,39–0,84). O escore MRNAS mostrou relação dose-resposta: comparados a indivíduos com 6 adequações, aqueles com 7 adequações (HR 0,52; IC95% 0,31–0,88), 8 adequações (HR 0,47; IC95% 0,30–0,73) e 9 nutrientes adequados (HR 0,49; IC95% 0,31–0,76), tiveram riscos significativamente menores de obesidade. Notavelmente, não houve associação significativa com a duração do sono ou demais condições cardiometabólicas. Em conclusão, tanto a ingestão de melatonina alimentar quanto a adequação dos nutrientes necessários à sua síntese endógena mostraram-se associados a menor risco de obesidade. Este estudo pioneiro sugere que estratégias de crononutrição focadas na densidade de micronutrientes podem estar relacionadas à regulação metabólica, contribuindo para a prevenção de doenças crônicas no Brasil. Esses achados ampliam o conhecimento sobre a relação entre dieta, melatonina e saúde metabólica e fornecem subsídios para futuras investigações e estratégias nutricionais voltadas à prevenção de doenças crônicas não transmissíveis. Its endogenous synthesis depends on precursors and cofactors such as tryptophan, B-complex vitamins, magnesium, zinc, and omega-3, and it can also be obtained through diet. This study evaluated the associations between dietary melatonin intake, adequacy of nutrients involved in endogenous melatonin synthesis (MRNAS score), and health outcomes in adults from the CUME+ Study (2016–2024). Dietary intake was assessed using a validated food frequency questionnaire (144 items), applying a novel melatonin database for 119 foods. Outcomes included obesity (Body Mass Index (BMI) 30 kg/m²), depression (self-reported medical diagnosis), both previously validated for this cohort, sleep duration (self-reported), as well as cardiometabolic conditions (hypertension, type 2 diabetes mellitus, dyslipidemia, metabolic syndrome, and obstructive sleep apnea), obtained through self-reported diagnoses and clinical/biochemical data. Covariates included sex, age, marital status, income, physical activity, sleep duration, screen time, use of weight-control medications, smoking, alcohol, caffeine, and ultra-processed food consumption, in addition to baseline BMI. Statistical analyses were conducted using Stata software (version 19), adopting a 5% significance level. Descriptive analyses included mean and frequency tests (Kruskal-Wallis, Mann-Whitney, and chi-square). Associations involved logistic and Poisson regression models with robust variance for prevalence, as well as Cox proportional hazards models for incidence, adjusted for potential confounders. As a result, daily consumption of coffee, beans/lentils, and rice was the main contributor to dietary melatonin intake (mean 25,554.7 ng/day).
