Impacto da ventosaterapia sobre a dor crônica nas costas em adultos e idosos: estudo quase-experimental

Abstract
A dor crônica nas costas é uma condição prevalente em adultos e idosos, com repercussões negativas na funcionalidade, qualidade de vida e uso de serviços de saúde. Nesse contexto, as Práticas Integrativas e Complementares em Saúde, como a ventosaterapia, têm se destacado como estratégias promissoras e de baixo custo para o manejo da dor musculoesquelética. O objetivo deste estudo foi avaliar o impacto da ventosaterapia seca, nas modalidades fixa e deslizante, sobre a dor crônica nas costas em adultos e idosos. Trata-se de um estudo quase-experimental, do tipo antes e depois, desenvolvido em um centro de reabilitação do Sistema Único de Saúde, com 92 participantes submetidos a cinco sessões semanais de ventosaterapia. As avaliações foram realizadas antes e após a intervenção, contemplando desfechos clínicos, funcionais e bioquímicos. Para análise dos dados foram aplicados os testes de Wilcoxon emparelhado e McNemar, com nível de significância de 5%. Evidenciou-se aumento significativo do limiar de dor à pressão nas regiões cervical e lombar, redução da intensidade da dor e melhora das incapacidades física, funcional e psicossocial relacionadas à dor. Houve ainda melhora do perfil oxidativo, com aumento da atividade das enzimas antioxidantes catalase e superóxido dismutase e da capacidade antioxidante total, além de redução do consumo de medicamentos e da procura por serviços de saúde em decorrência da dor. A ventosaterapia foi bem avaliada pelos participantes, com relatos de satisfação e baixos índices de efeitos adversos. Conclui-se que a ventosaterapia demonstrou impacto positivo no tratamento da dor crônica nas costas, configurando-se como uma intervenção efetiva, segura e aplicável na prática clínica. Chronic back pain is prevalent among adults and older adults and has negative repercussions for functionality, quality of life, and healthcare utilization. In this context, Integrative and Complementary Practices in Health, such as cupping therapy, have emerged as promising, low-cost strategies for managing musculoskeletal pain. The objective of this study was to evaluate the impact of dry cupping, in fixed and sliding modalities, on chronic back pain in adults and older adults. This was a quasi-experimental, before-and-after study conducted at a rehabilitation center of the Sistema Único de Saúde [Unified Health System (SUS)], with 92 participants who underwent five weekly cupping sessions. Assessments were performed before and after the intervention and encompassed clinical, functional, and biochemical outcomes. Data were analyzed using the paired Wilcoxon test and McNemar’s test, with a 5% significance level. There was a significant increase in pressure pain threshold in the cervical and lumbar regions, a reduction in pain intensity, and improvements in physical, functional, and psychosocial disabilities related to pain. There was also improvement in the oxidative profile, with increased activity of the antioxidant enzymes catalase and superoxide dismutase and increased total antioxidant capacity, in addition to reduced use of medications and decreased healthcare service utilization due to pain. Cupping therapy was well rated by participants, with high satisfaction and low rates of adverse effects. In conclusion, cupping therapy demonstrated a positive impact on the treatment of chronic back pain, and it appears to be an effective, safe, and clinically applicable intervention.
