Exposição vertical ao zika vírus: associações com habilidades funcionais e estado nutricional em crianças em idade escolar

Abstract
A propagação do Zika vírus (ZIKV) no Brasil, em 2015, acarretou uma situação epidemiológica alarmante, pelo aumento do número de notificações da exposição ao vírus no período gestacional e associação da infecção com complicações neonatais e infantis. Reconhecendo o neutropismo agressivo desse vírus e possibilidades de comprometimento do estado nutricional e desenvolvimento dessas crianças, este estudo objetivou avaliar a associação da exposição vertical ao ZIKV e da microcefalia com prejuízos das habilidades funcionais, no estado nutricional e no risco cardiometabólico em crianças. Trata-se de uma coorte retrospectiva cujos sujeitos foram crianças residentes em Belo Horizonte e Região Metropolitana nascidas durante a epidemia causada pelo ZIKV (2015-2017). As crianças foram agrupadas em: 1. Com exposição vertical ao ZIKV e com microcefalia (n=14); 2. Com exposição vertical ao ZIKV e sem microcefalia (n=41); e 3. Sem exposição vertical ao ZIKV e sem microcefalia (n=38). Considerou-se exposição vertical ao ZIKV a partir da detecção do RNA viral (RT-PCR) e sorologia dos anticorpos IgM para ZIKV ou diagnóstico clínico epidemiológico. Para verificar o desenvolvimento das habilidades funcionais foi utilizado o Inventário de Avaliação Pediátrica de Incapacidade (PEDI). O estado nutricional foi avaliado por meio de medidas antropométricas, incluindo os indicadores peso/idade, estatura/idade e índice de massa corporal para idade (IMC/Idade). O risco cardiometabólico foi analisado por meio da aferição do perímetro da cintura e do pescoço, bem como por exames bioquímicos, incluindo as dosagens séricas de vitamina A, vitamina D, ferro sérico, ferritina, colesterol total, HDL-c, LDL-c e triglicerídeos). O estudo foi aprovado pelo Comitê de Ética em Pesquisa com Seres Humanos da Universidade Federal de Viçosa e Secretaria Municipal de Saúde de Belo Horizonte (código de protocolos 2.705.484 e 3.526.111). Modelos de regressão linear e de Poisson, com variância robusta, ajustados por potenciais fatores de confusão, bem como análise de interação, foram realizados para avaliar a associação da exposição vertical ao ZIKV e da microcefalia com os desfechos relacionados às habilidades funcionais, estado nutricional e risco cardiometabólico, considerando nível de significância de 5% para todos os testes estatísticos. Os resultados demonstraram que as crianças com exposição vertical ao ZIKV apresentaram menores escores-z de peso/idade e estatura/idade, além de atrasos no desenvolvimento das habilidades funcionais. Entretanto, foram observadas maiores concentrações de vitamina D sérica nesse grupo. Além disso, crianças expostas ao ZIKV apresentaram maior prevalência de HDL-c baixo em comparação às não expostas. O IMC para idade esteve inversamente associado ao HDL-c baixo. Além disso, foram observadas interações significativas entre exposição intrauterina ao Zika vírus e o IMC para idade em relação ao colesterol total e triglicerídeos elevados. Entre as crianças expostas, o IMC para idade apresentou forte associação com aumento do colesterol total. Houve interação significativa entre o perímetro da cintura e exposição ao Zika vírus para colesterol total, com associação mais forte entre as crianças expostas. O perímetro do pescoço esteve positivamente associado ao colesterol total, LDL-c e triglicerídeos entre as crianças expostas. Conclui-se que o monitoramento contínuo por equipe multidisciplinar é relevante para essas crianças em situação de vulnerabilidade. Os achados reforçam a necessidade de mais estudos com seguimento a longo prazo para promover o acompanhamento do crescimento e desenvolvimento dessas crianças, bem como a compreensão dos impactos cardiometabólicos da exposição vertical ao ZIKV ao longo da vida. Recognizing the virus’s strong neurotropism and the potential for impairment of nutritional status and development in affected children, this study aimed to evaluate the association between vertical exposure to ZIKV and microcephaly with impairments in functional abilities, nutritional status, and cardiometabolic risk in children. This is a retrospective cohort study involving children residing in Belo Horizonte and its Metropolitan Region, born during the ZIKV epidemic (2015–2017). The children were grouped as follows: (1) vertically exposed to ZIKV with microcephaly (n=14); (2) vertically exposed to ZIKV without microcephaly (n=41); and (3) non-exposed to ZIKV without microcephaly (n=38). Vertical exposure to ZIKV was defined based on detection of viral RNA (RT-PCR), IgM antibody serology for ZIKV, or a clinical-epidemiological diagnosis. Functional abilities were assessed using the Pediatric Evaluation of Disability Inventory (PEDI). Nutritional status was evaluated using anthropometric measurements, including weight-for-age, height-for-age, and body mass index-for-age (BMI-for-age). Cardiometabolic risk was assessed through waist and neck circumference measurements, as well as biochemical tests, including serum levels of vitamin A, vitamin D, iron, ferritin, total cholesterol, HDL-c, LDL-c, and triglycerides. The study was approved by the Research Ethics Committees of the Federal University of Viçosa and the Municipal Health Department of Belo Horizonte (protocol numbers 2,705,484 and 3,526,111). Linear regression and Poisson regression models with robust variance, adjusted for potential confounders, as well as interaction analyses, were performed to assess associations between vertical exposure to ZIKV and microcephaly with functional, nutritional, and cardiometabolic outcomes, considering a significance level of 5% for all statistical tests. The results showed that children with vertical exposure to ZIKV had lower weight-for- age and height-for-age z-scores, as well as delays in functional skill development. However, higher serum vitamin D concentrations were observed in this group. In addition, ZIKV-exposed children showed a higher prevalence of low HDL-c compared to non-exposed children. BMI-for-age was inversely associated with low HDL-c. Significant interactions were observed between intrauterine ZIKV exposure and BMI-for-age regarding elevated total cholesterol and triglycerides. Among exposed children, BMI-for-age showed a strong association with increased total cholesterol. A significant interaction was also found between waist circumference and ZIKV exposure for total cholesterol, with stronger associations among exposed children. Neck circumference was positively associated with total cholesterol, LDL-c, and triglycerides among exposed children. It is concluded that continuous monitoring by a multidisciplinary healthcare team is essential for these vulnerable children. The findings highlight the need for further long-term follow-up studies to support growth and developmental monitoring and to improve understanding of the cardiometabolic impacts of vertical exposure to ZIKV throughout life.
