Notificações de eventos adversos: a realidade de um hospital de ensino brasileiro

Resumo
À presente pesquisa teve como objetivo compreender a percepção dos enfermeiros em relação às
notificações de eventos adversos no âmbito hospitalar e como se dão suas práticas em relação a esse
processo. Pesquisa descritiva de natureza qualitativa que contou com a participação de 24
enfermeiros (assistenciais e coordenadores de setor) que atuam em um hospital escola
brasileiro. A coleta de dados foi realizada de maio a novembro de 2018, através de entrevistas
com roteiro semiestruturado, contendo questões abertas. A fim de complementar e elucidar as
entrevistas realizadas, utilizou-se a pesquisa documental através do acesso as notificações
realizadas, permitindo traçar um perfil das mesmas. A pesquisa obteve aprovação do Comitê
de Ética da universidade a qual se vincula a pesquisa. O referencial metodológico escolhido
foi a Teoria Fundamentada em dados, onde a coleta e análise dos dados se dá de forma
concomitante. Os produtos finais da presente dissertação foram a confecção de dois artigos:
um original e um artigo de revisão integrativa. O artigo original permitiu conhecer aspectos
importantes relacionados a percepção e as práticas dos enfermeiros em relação ao processo de
notificação dos EA. Os participantes conseguem identificar os EA quando eles ocorrem, têm
discernimento de quais são passíveis de notificação, reconhecem que as notificações
contribuem para a segurança do paciente e têm a capacidade para subsidiar melhoras na
assistência em saúde. Em contrapartida, demonstram fragilidades relacionadas as notificações,
e não estão habituados a notificar em sua prática diária visto o baixo número de notificações
realizadas. A literatura traz diversos benefícios das notificações de EA para a assistência à
saúde. Através delas é possível conhecer os EA que ocorrem nas instituições, bem como suas
causas, subsidiando ações com o intuito de prevenir para que os mesmos não voltem a ocorrer
e, consequentemente, não atinjam o paciente, culminando assim na segurança desse. Contudo,
dois grandes desafios ainda enfrentados são: a não notificação por outras classes profissionais
além da enfermagem e a subnotificação.
The present research aimed to understand nurses' perceptions regarding notifications of adverse events in hospitals and how their practices are related to this process. Data collection was performed from May to November 2018, through interviews with semi-structured script contamimg open questions. In order to complement and elucidate the interviews, the documentary research was used through the access to the notifications performed, allowing to draw a profile of them. The research was approved by the University Ethics Committee to which the research 1s linked. The methodological framework chosen was the Grounded Theory, where data collection and analysis occur concurrently. The final products of this dissertation were the writing of two articles: an original and an integrative review article. The original article allowed us to know important aspects related to nurses perception and practices regarding the notification process of AEÉs. Participants are able to identify AÉEs when they occur, have insight into which ones are notifiable, recognize that notifications contribute to patient safety, and have the ability to subsidize improvements im health care. In contrast, they show weaknesses related to notifications, and are not accustomed to notify im their daily practice given the low number of notifications made. The literature brings several benefits of AE notifications for health care. Through them 1t 1s possible to know the AEs that occur in the institutions, as well as their causes, subsidizing actions im order to prevent them from happening agam and, consequently, do not reach the patient, thus culminating im the patient's safety. However, two major challenges still faced are: non-notification by other professional classes besides nursing and underreporting
