Item A pressão arterial está associada a indicadores de adiposidade corporal em crianças de quatro a sete anos(Revista Portuguesa de Cardiologia, 2017-07-24) Vieira, Sarah Aparecida; Fonseca, Poliana Cristina de Almeida; Andreoli, Cristiana Santos; Pereira, Patrícia Feliciano; Hermsdorff, Helen Hermana Miranda; Ribeiro, Andréia Queiroz; Priore, Silvia Eloiza; Franceschini, Sylvia do Carmo CastroEstudos feitos com adultos têm demonstrado associação entre aumento da adiposidade corporal e hipertensão arterial, mas ainda são escassos estudos que avaliaram essa associação na infância. O objetivo deste estudo foi investigar a associação entre pressão arterial e indicadores de adiposidade corporal em crianças, controlada por variáveis de nascimento, sociodemográficas, de estilo de vida e dietéticas.Estudo transversal com amostra de 399 crianças de quatro a sete anos. A variável dependente foi a pressão arterial, aferida de acordo com protocolo estabelecido pela VII Diretriz Brasileira de Hipertensão Arterial. As variáveis explicativas do estudo foram os índices relação cintura‐estatura (RCE) e índice de massa corporal (IMC). A regressão linear múltipla foi usada para avaliar a associação independente entre indicadores de adiposidade e pressão arterial, ajustada por variáveis de nascimento, sociodemográficas, estilo de vida e alimentação das crianças. Considerou‐se como significância estatística α = 5%.O IMC/I e a RCE correlacionaram‐se positivamente com a pressão arterial sistólica e diastólica. Após as análises múltiplas, os maiores valores de IMC (modelo 1) e RCE (modelo 2) se associaram ao aumento da pressão arterial sistólica e diastólica.O aumento da adiposidade corporal, avaliada pelo IMC e pela RCE, associou‐se ao aumento da pressão arterial sistólica e diastólica entre as crianças avaliadas. Assim, sugere‐se o uso do IMC e da RCE na avaliação nutricional de crianças, para screening de alterações na pressão arterial e outros fatores de risco cardiometabólico nessa população.Item Analysis of the national school feeding program in the municipality of Viçosa, state of Minas Gerais(Revista de Saúde Pública, 2017-03-21) Rocha, Naruna Pereira; Filgueiras, Mariana De Santis; Albuquerque, Fernanda Martins de; Milagres, Luana Cupertino; Castro, Ana Paula Pereira; Silva, Mariane Alves; Costa, Glauce Dias da; Priore, Silvia Eloiza; Novaes, Juliana Farias deTo analyze the implementation of the Brazilian National School Feeding Program as a food and nutritional security policy in public schools. This a cross-sectional study, with a quantitative and qualitative approach, carried out with 268 schoolchildren aged eight to nine years from the public school system of Viçosa, state of Minas Gerais, Brazil, in 2015. Interviews were carried out using semi-structured questionnaires with the children, parents, cooks, nutritionists, trainer of the Technical Assistance and Rural Extension Company, and president of the School Feeding Council. In order to analyze the implementation of the National School Feeding Program in Viçosa, we evaluated the direct weighing of the food served in the schools using mechanical balances with a capacity of up to 10 kg and the perception of the social players involved in the implementation of the National School Feeding Program. The children were questioned about the acceptance of and adherence to the food offered, in addition to the habit of bringing food from home. Parents reported knowledge about the School Feeding Program and Council. The qualitative analysis consisted of content analysis and quantitative analysis using the chi-square test, Fisher’s exact test, and Mann-Whitney test. We adopted the statistical significance of 5% for quantitative analysis. Children reported low adherence to the school feeding program and most of them used to bring food from home. Irregularities were identified in the implementation of the National School Feeding Program, such as: inadequate number of nutritionists, suspension of Council meetings, inadequate infrastructure in the areas of preparation and distribution of meals, lack of training of cooks, lack of nutritional adequacy of the food offered, and lack of actions on food and nutritional education. The Program complied with the recommendations for purchasing food from family farms. The National School Feeding Program presented many irregularities in Viçosa. It is important to monitor the problems identified for better reformulation and planning of the Program, in order to guarantee the food and nutritional security of the children served.Item Anemia em gestantes brasileiras antes e após a fortificação das farinhas com ferro(Revista de Saúde Pública, 2011-10-14) Priore, Silvia Eloiza; Fujimori, Elizabeth; Sato, Ana Paula Sayuri; Szarfarc, Sophia Cornbluth; Veiga, Gloria Valeria da; Oliveira, Valterlinda Alves de; Colli, Célia; Moreira-Araújo, Regilda Saraiva dos Reis; Arruda, Ilma Kruze Grande de; Uchimura, Taqueco Teruya; Brunken, Gisela Soares; Yuyama, Lucia Kiyoko Ozaki; Muniz, Pascoal Torres; Tsunechiro, Maria Alice; Frazão, Andréa das Graças Ferreira; Passoni, Cynthia R Matos Silva; Araújo, Claudia Regina Marchiori AntunesComparar prevalência de anemia e valores de hemoglobina (Hb) em gestantes brasileiras, antes e após a fortificação das farinhas com ferro.Estudo de avaliação de painéis repetidos, desenvolvido em serviços públicos de saúde de municípios das cinco regiões brasileiras. Dados retrospectivos foram obtidos de 12.119 prontuários de gestantes distribuídas em dois grupos: antes da fortificação, com parto anterior a junho de 2004, e após a fortificação, com última menstruação após junho de 2005. Anemia foi definida como Hb < 11,0 g/dL. Valores de Hb/idade gestacional foram avaliados segundo dois referenciais da literatura. Foram utilizados teste qui-quadrado, t de Student e regressão logística, com nível de 5% de significância.Na amostra total, anemia caiu de 25% para 20% após fortificação (p < 0,001), com médias de Hb significativamente maiores no grupo "após" (p < 0,001). Observaram-se, entretanto, diferenças regionais importantes: reduções significativas nas regiões Nordeste (37% para 29%) e Norte (32% para 25%), onde as prevalências de anemia eram elevadas antes da fortificação, e reduções menores nas regiões Sudeste (18% para 15%) e Sul (7% para 6%), onde as prevalências eram baixas. Os níveis de Hb/idade gestacional de ambos os grupos se mostraram discretamente mais elevados nos primeiros meses, porém bem mais baixos após o terceiro ou quarto mês, dependendo da referência utilizada para comparação. Análise de regressão logística mostrou que grupo, região geográfica, situação conjugal, trimestre gestacional, estado nutricional inicial e gestação anterior associaram-se com anemia (p < 0,05).A prevalência de anemia diminuiu após a fortificação, porém continua elevada nas regiões Nordeste e Norte. Embora a fortificação possa ter tido papel nesse resultado favorável, há que se considerar a contribuição de outras políticas públicas implementadas no período estudado.Item Anthropometric indices to identify metabolic syndrome and hypertriglyceridemic waist phenotype: a comparison between the three stages of adolescence(Revista Paulista de Pediatria, 2014-10-14) Pereira, Patrícia Feliciano; Faria, Franciane Rocha de; Faria, Eliane Rodrigues de; Hermsdorff, Helen Hermana Miranda; Peluzio, Maria do Carmo Gouveia; Franceschini, Sylvia do Carmo Castro; Priore, Silvia EloizaTo determine the prevalence of metabolic syndrome (MS) and the hypertriglyceridemic waist phenotype (HW) in a representative adolescent sample; as well as to establish which anthropometric indicator better identifies MS and HW, according to gender and adolescent age. This cross sectional study had the participation of 800 adolescents (414 girls) from 10‐19 years old. Anthropometric indicators (body mass index, waist perimeter, waist/stature ratio, waist/hip ratio, and central/peripheral skinfolds) were determined by standard protocols. For diagnosis of MS, the criteria proposed by de Ferranti et al. (2004) were used. HW was defined by the simultaneous presence of increased waist perimeter (>75th percentile for age and sex) and high triglycerides (>100 mg/dL). The ability of anthropometric indicators was evaluated by Receiver Operating Characteristic curve. The prevalence of MS was identical to HW (6.4%), without differences between genders and the adolescence phases. The waist perimeter showed higher area under the curve for the diagnosis of MS, except for boys with 17‐19 years old, for whom the waist/stature ratio exhibited better performance. For diagnosing HW, waist perimeter also showed higher area under the curve, except for boys in initial and final phases, in which the waist/stature ratio obtained larger area under the curve. The central/peripheral skinfolds had the lowest area under the curve for the presence of both MS and HW phenotype. The waist perimeter and the waist/stature showed a better performance to identify MS and HW in both genders and in all three phases of adolescence.Item Aplicabilidade de equações na avaliação da composição corporal da população brasileira(Revista de Nutrição, 2006-05) Rezende, Fabiane Aparecida Canaan; Rosado, Lina Enriqueta Frandsen Paez Lima; Priore, Silvia Eloiza; Franceschin, Sylvia do Carmo CastroO excesso de gordura corporal está, freqüentemente, associado a alterações metabólicas importantes e às doenças crônicas, como diabetes, hipertensão arterial e dislipidemias. A disponibilidade de métodos que permitam estimar a composição corporal é fundamental para avaliação de atletas e do estado de saúde de indivíduos. Entre os métodos existentes, as medidas de dobras cutâneas são habitualmente utilizadas na população brasileira, devido à sua facilidade e relativa precisão em estimar a gordura corporal. Entretanto, existem limitações quanto à utilização desse método, que podem implicar em erros de estimativas da gordura corporal importantes. A proposta deste trabalho é descrever os estudos que validaram essas equações e os principais fatores que influenciam a acurácia das mesmas na população em geral. Também são discutidos aspectos quanto à aplicabilidade dessas equações na população brasileira.Item Body composition, biochemical and clinical changes of adolescents with excessive adiposity(Arquivos Brasileiros de Cardiologia, 2012-04-12) Serrano, Hiara Miguel Stanciola; Carvalho, Gisele Queiroz; Pereira, Patrícia Feliciano; Peluzio, Maria do Carmo Gouveia; Franceschini, Sylvia do Carmo Castro; Priore, Silvia EloizaAdolescents with excess body fat and eutrophic had the same metabolic changes expected in obese individuals. To evaluate body composition, anthropometric changes, biochemical and clinical characteristics of female adolescents. A total of 113 adolescents from public schools in Viçosa, MG, divided into three groups: group 1 - consisting of eutrophic adolescents with excess body fat; group 2 - eutrophic with body fat within normal limits; and group 3 - with excess weight and body fat. Weight, height, waist and hip circumference, blood pressure were measured. The body mass index (BMI) and waist-hip ratio were calculated. The percentage of body fat was obtained by bioelectrical impedance horizontally, following its own protocol for this evaluation. The assessment of the percentage of body fat and biochemistry was performed after 12 hours of fasting, and analyzed the lipid profile, blood glucose and insulin, homocysteine, leptin and C-reactive protein. Insulin resistance was calculated by HOMA index. The group of eutrophic adolescents, with higher adiposity, behaved in relation to blood pressure, HDL and glucose levels, similarly to adolescents who are overweight. It can be seen that the HOMA index, insulin and leptin increased with increasing body fat. More than half of adolescents had total cholesterol and CRP levels above recommended levels. The most obvious metabolic disorder related to the lipid profile for both groups studied. Excess adiposity in normal weight adolescents may be related to clinical and biochemical changes similar to those found in adolescents who are overweight.Item Body dissatisfaction, physical activity, and sedentary behavior in female adolescents(Revista Paulista de Pediatria, 2018-10) Miranda, Valter Paulo Neves; Morais, Núbia Sousa de; Faria, Eliane Rodrigues de; Amorim, Paulo Roberto dos Santos; Marins, João Carlos Bouzas; Franceschini, Sylvia do Carmo Castro; Teixeira, Paula Costa; Priore, Silvia EloizaTo evaluate the association of body image with physical activity level, body composition, and sedentary behavior (SB) of female adolescents. Exploratory cross‑sectional study conducted with 120 female adolescents aged between 14‑19 years, from the city of Viçosa, Minas Gerais, Southeast Brazil. Body image was evaluated with a Body Silhouette Scale (BSS) and a Body Shape Questionnaire (BSQ). Weight, height, and waist circumference values were analyzed, as well as the waist‑to‑height ratio and body fat percentage. The physical activity level (PAL) was assessed by 24‑hour Physical Activity Recall and SB by screen time, that is, time spent in front of a TV, playing video game, on the computer and using tablets, and, separately, the cell phone time. Mean age was 16.5± 1.5 years, and most adolescents were eutrophic (77.6%), sedentary/low PAL (84.2%), with high screen time (85.2%) and cell phone time (58.7%). Body dissatisfaction was stated in 40.6% of BSQ and 45.8% of BSS evaluations. Body distortion was identified in 52.9% of participants. All body composition measures, along with cell phone time and PAL, were associated with body dissatisfaction, the more active adolescents presenting higher levels of dissatisfaction. This study concluded that female adolescents with higher cell phone time also present higher body dissatisfaction, as well as the most physically active ones. All body composition measurements were associated with body dissatisfaction, mainly body mass index, waist circumference, and waist‑to‑height ratio.Item Body fat equations and electrical bioimpedance values in prediction of cardiovascular risk factors in eutrophic and overweight adolescents(International Journal of Endocrinology, 2013) Faria, Franciane Rocha; Faria, Eliane Rodrigues; Cecon, Roberta Stofeles; Barbosa Júnior, Djalma Adão; Franceschini, Sylvia do Carmo Castro; Peluzio, Maria do Carmo Gouveia; Ribeiro, Andréia Queiroz; Lira, Pedro Israel Cabral; Cecon, Paulo Roberto; Priore, Silvia EloizaTheaimof this study was to analyze body fat anthropometric equations and electrical bioimpedance analysis (BIA) in the prediction of cardiovascular risk factors in eutrophic and overweight adolescents. 210 adolescents were divided into eutrophic group (G1) and overweight group (G2).The percentage of body fat (% BF) was estimated using 10 body fat anthropometric equations and 2 BIA. We measured lipid profiles, uric acid, insulin, fasting glucose, homeostasis model assessment-insulin resistance (HOMA-IR), and blood pressure. We found that 76.7% of the adolescents exhibited inadequacy of at least one biochemical parameter or clinical cardiovascular risk. Higher values of triglycerides (TG) (P=0.001), insulin, and HOMA-IR (P< 0.001) were observed in the G2 adolescents. Inmultivariate linear regression analysis, the % BF fromequation (5) was associated with TG, diastolic blood pressure, and insulin in G1. Among the G2 adolescents, the % BF estimated by (5) and (9) was associated with LDL, TG, insulin, and the HOMA-IR. Body fat anthropometric equations were associated with cardiovascular risk factors and should be used to assess the nutritional status of adolescents. In this study, equation (5) was associated with a higher number of cardiovascular risk factors independent of the nutritional status of adolescents.Item Café com Agroecologia: integrando conhecimentos(Revista ELO - Diálogos em Extensão, 2017-10-24) Prates Júnior, Paulo; Pereira, Adalgisa de Jesus; Santana, Felipe Carvalho; Santos, Lidiane Figueiredo; Carmo, Davi Lopes; Priore, Silvia Eloiza; Casali, Vicente Wagner DiasA Agroecologia é uma ciência que dialoga com diferentes áreas de conhecimentos via metodologias participativas. Em contexto de troca de saberes surgiu o "Café com Agroecologia", promovido pelo Programa de Pós-graduação em Agroecologia da Universidade Federal de Viçosa, com encontros mensais. Agregam estudantes do ensino fundamental e médio, graduação e pós-graduação, professores, técnicos, produtores rurais e demais interessados pela Agroecologia. O encontro normalmente é iniciado pela fala do facilitador/ palestrante sobre o tema a ser discutido. Em seguida, são promovidos posicionamentos e questionamentos que, comumente, são direcionados ao facilitador. O espaço é aberto à troca, preparo e degustação de alimentos e bebidas diversas, visando valorizar a socioagrobiodiversidade e a confraternização entre os participantes. O artigo visa propor reflexões sobre a popularização da Agroecologia, como também divulgar, compreender e compartilhar o funcionamento das vivências ocorridas nos encontros.Item Circunferência da cintura como indicador de gordura corporal e alterações metabólicas em adolescentes: comparação entre quatro referências(Revista da Associação Médica Brasileira, 2010) Peluzio, Maria do Carmo Gouveia; Franceschini, Silvia do Carmo Castro; Priore, Silvia Eloiza; Pereira, Patrícia Feliciano; Serrano, Hiara Miguel Stanciola; Carvalho, Gisele Queiroz; Lamounier, Joel AlvesAvaliar a validade diagnóstica de quatro tabelas de referência para circunferência da cintura em adolescentes do sexo feminino para detecção de alterações lipídicas, hiperinsulinemia, homeostasis model assessment (HOMA) elevado, hiperleptinemia e elevada adiposidade corporal. Avaliadas 113 adolescentes com idade entre 14 e 19 anos, provenientes de escolas públicas de Viçosa (MG). Em amostras de sangue foram dosados colesterol total, LDL, HDL, triglicerídeos, insulina e leptina. Determinado percentual de gordura corporal através de bioimpedância elétrica tetrapolar. Pela medida de menor diâmetro abdominal foi determinada a circunferência da cintura do abdômen e calculados valores de sensibilidade, especificidade, valor preditivo positivo e negativo. Foram elaboradas tabelas de contingência de classificação de circunferência da cintura em adolescentes para quatro critérios: Freedman et al., 1999; Taylor et al., 2000; McCarthy et al., 2001; e Moreno et al., 2007. Valores de sensibilidade em geral foram baixos para as referências avaliadas, sendo os maiores obtidos para a de McCarthy et al. Ao contrário, as especificidades foram altas, principalmente para a tabela de Freedman et al. Os valores preditivos positivos foram mais relevantes para colesterol total e percentual de gordura corporal.Os pontos de corte para circunferência da cintura de McCarthy et al. demonstraram-se os mais adequados para avaliações populacionais. A proposta de Freedman et al. por apresentar maior especificidade, é útil para uso clínico e pode substituir a realização de exames de custo elevado que em muitos locais não se encontram ao alcance dos profissionais de saúde, como leptina e insulina.Item Circunferência da cintura e relação cintura/estatura: úteis para identificar risco metabólico em adolescentes do sexo feminino?(Revista Paulista de Pediatria, 2011-09) Pereira, Patrícia Feliciano; Serrano, Hiara Miguel S.; Carvalho, Gisele Queiroz; Peluzio, Maria do Carmo G.; Franceschini, Sylvia do Carmo C.; Priore, Silvia Eloiza; Lamounier, Joel AlvesAvaliar se a medida da circunferência da cintura e a relação cintura/estatura (RCE) são preditoras de fatores de risco cardiovasculares em adolescentes do sexo feminino. Avaliaram-se 113 adolescentes de 14 a 19 anos quanto à antropometria (peso, estatura e circunferência da cintura), parâmetros bioquímicos e clínicos (colesterol total, LDL-C, HDL-C, triglicerídeos, glicemia de jejum, insulina, homeostasis model assessment to assess insulin resistance - HOMA-IR, leptina, homocisteína e pressão arterial). Considerou-se como ponto de corte de obesidade abdominal valores de cintura e RCE>percentil 90. As adolescentes com obesidade abdominal apresentaram valores significantemente maiores de triglicerídeos (exceto para a RCE), insulina, HOMA-IR, leptina, pressão arterial sistólica e diastólica; o HDL-C foi mais baixo no grupo com cintura >percentil 90, porém sem significância estatística (p=0,052). O presente estudo demonstrou que a cintura e a RCE são medidas úteis para identificar adolescentes do sexo feminino com maior risco cardiovascular; contudo, a circunferência da cintura, isoladamente, apresentou melhor desempenho.Item Colônia de férias: um espaço social a ser explorado pela saúde?(ELO Diálogos de Extensão, 2014-07) Souza, Ramon Augusto Ferreira de; Nunes, Edilene do Nascimento; Lima, Jércica Lopes de; Vieira, Nayara Cassimiro; Priore, Silvia Eloiza; Silva, Renato Pereira daA educação em saúde pode contribuir de maneira significativa para a qualidade de vida de uma população, principalmente, das crianças, mais vulneráveis e expostas a situações que podem lhe trazer riscos. O objetivo deste artigo foi demonstrar a experiência desenvolvida na Colônia de Férias da Universidade Federal de Viçosa (UFV), verificando a viabilidade do aproveitamento de espaços sociais não-formais para a promoção de saúde. Palestras e atividades dinâmicas sobre higiene corporal, educação nutricional, saúde bucal e atividades físicas foram realizadas por acadêmicos do curso de enfermagem, nutrição e educação física da UFV. Exames odontológicos para detecção de cárie dentária também foram realizados. Face ao grande envolvimento do público participante, e identificação de crianças com necessidade de tratamento odontológico, evidenciou-se que a Colônia de Férias/UFV, além de um espaço para lazer das crianças pode se configurar como um espaço para educação em saúde para estas crianças.Item Contribuição dos domínios físico, social, psicológico e ambiental para a qualidade de vida global de idosos(Revista de Psiquiatria do Rio Grande do Sul, 2006-01) Pereira, Renata Junqueira; Cotta, Rosângela Minardi Mitre; Franceschini, Sylvia do Carmo Castro; Ribeiro, Rita de Cássia Lanes; Sampaio, Rosana Ferreira; Priore, Silvia Eloiza; Cecon, Paulo RobertoEste estudo investigou a contribuição de cada domínio da qualidade de vida (físico, social, psicológico e ambiental) na qualidade de vida global e em que extensão esses domínios explicam a qualidade de vida global de idosos residentes no município de Teixeiras, na Região Sudeste do Brasil. Trata-se de um estudo observacional de corte transversal, sendo a amostragem aleatória estratificada por sexo, uso do Programa Saúde da Família e microárea de saúde (n = 211, indivíduos com idade igual ou superior a 60 anos), representando 14,28% do total de idosos do município em estudo. O instrumento utilizado foi o Word Health Organization Quality of Life Instrument Bref (WHOQOL-Bref), proposto pela Organização Mundial de Saúde, aplicado por um único entrevistador, devidamente treinado. As análises de regressão linear mostraram que nenhuma das variáveis sociodemográficas interferiu significativamente no domínio global da qualidade de vida, e, entre os quatro domínios, o que mais explicou a qualidade de vida global foi o físico, seguido do ambiental e do psicológico. O domínio social não mostrou contribuição significativa na qualidade de vida global. As possíveis explicações para os resultados encontrados foram discutidas.Item Correlação entre variáveis de composição corporal e metabólica em adolescentes do sexo feminino(Arquivos Brasileiros de Cardiologia, 2008-10-24) Faria, Eliane Rodrigues de; Franceschini, Sylvia do Carmo Castro; Peluzio, Maria do Carmo Gouveia; Sant’Ana, Luciana Ferreira da Rocha; Priore, Silvia EloizaAnalysis of metabolic alterations and body composition in adolescents.To study the correlation of serum levels of lipids, glycemia, insulin, homocysteine, HOMA-IR and blood pressure among themselves and with body variables. Data concerning glycemia, total cholesterol and fractions (LDL, HDL and VLDL), triacylglycerols, insulin, homocysteine and blood pressure were measured in 100 adolescents at the age range of 14 to 17 years, who had already had menarche and attended the public schools in Vicosa, state of Minas Gerais, Brazil. The percentage of body fat (%BF) was evaluated by horizontal bioimpedance. In relation to nutritional status, 83%, 11% and 6%, respectively were eutrophic (EU), presented overweight/overweight risk (OW/OR) or low weight (LW) (CDC/NCHS, 2000) and 61% presented high percentage of BF. Total cholesterol presented the highest percentage of inadequacy (57%), followed by HDL (50%), LDL (47%) and triacylglycerols (22%). Inadequacy in 11%, 9%, 5% and 4% were observed in relation to insulin resistance, insulin, blood pressure and glycemia, respectively. For total cholesterol, insulin, HOMA-IR and nutritional state, OW/OR>LW (p<0.05). For body composition and nutritional status, OW/OR>EU>LW (p <0.001). Some positive and strong correlations were found between BMI and the anthropometrical measures estimating the % of total BF, as well as central distribution, except for the waist/hip ratio. The %BF was correlated with insulin levels (r=0.303; p <0.001) and HOMA-IR (r=0.281; p<0.001). Longitudinal study based on a birth cohort in Viçosa, MG, Southeastern Brazil. In 2011/2012, 168 new mothers accessing the public health network were followed. Three interviews, at 30, 60, and 120 days postpartum, with the new mothers were conducted. Exclusive breastfeeding abandonment was analyzed in the first, second, and fourth months after childbirth. The Edinburgh Postnatal Depression Scale was applied to identify depressive symptoms in the first and second meetings, with a score of ≥ 12 considered as the cutoff point. Socioeconomic, demographic, and obstetric variables were investigated, along with emotional conditions and the new mothers’ social network during pregnancy and the postpartum period. The prevalence of exclusive breastfeeding abandonment at 30, 60, and 120 days postpartum was 53.6% (n = 90), 47.6% (n = 80), and 69.6% (n = 117), respectively, and its incidence in the fourth month compared with the first was 48.7%. Depressive symptoms and traumatic delivery were associated with exclusive breastfeeding abandonment in the second month after childbirth. In the fourth month, the following variables were significant: lower maternal education levels, lack of homeownership, returning to work, not receiving guidance on breastfeeding in the postpartum period, mother’s negative reaction to the news of pregnancy, and not receiving assistance from their partners for infant care. Psychosocial and sociodemographic factors were strong predictors of early exclusive breastfeeding abandonment. Therefore, it is necessary to identify and provide early treatment to nursing mothers with depressive symptoms, decreasing the associated morbidity and promoting greater duration of exclusive breastfeeding. This is a cohort study with 240 children evaluated in four stages. Variables of birth, eating habits of the child, mothers’ breast-feeding difficulty and pacifier use were investigated. Children’s weight gain rate (grams/day) and size gain (cm/month) were measured in all assessments and compared according to the variables of interest. In the first month, weight gain rate of children born by cesarean section was smaller. By the second month, the growth rate (weight and size gain) was higher among children who were exclusively or predominantly breastfed and lower among those who consumed infant formula. Children of mothers who reported difficulty to breastfeed showed a lower growth rate until the second month. Children age four months who consumed porridge had lower weight and size gain rate. To assess the reproducibility of different measurements of the sagittal abdominal diameter and the waist perimeter and analyze the discriminating power of the measurements when predicting insulin resistance. A total of 190 adult males were studied. The sagittal abdominal diameter (smallest girth, larger abdominal diameter, umbilical level and midpoint between the iliac crests) and the waist perimeter (umbilical level, smallest girth, immediately above the iliac crest and midpoint between the iliac crest and the last rib) were measured at four different sites. Insulin resistance was assessed by the homeostasis model of assessment-insulin resistance (HOMA-IR) index. All measurements presented an intraclass correlation of 0.986-0.999. The sagittal abdominal diameter measured at the smallest girth (r=0.482 and AUC=0.739±0.049) and the waist perimeter measured at the midpoint between the last rib and the iliac crest (r=0.464 and AUC=0.746±0.05) presented the highest correlations with the HOMA-IR and the best discriminating power for HOMA-IR according to the ROC analysis (p<0.001). The sagittal abdominal diameter and waist perimeter showed to be highly reproducible and the sagittal abdominal diameter (smallest girth) and waist perimeter (midpoint between the iliac crest and the last rib) presented the best performance when predicting HOMA-IR. Further studies in other groups of the Brazilian population must be carried out to allow the use of these indicators of insulin resistance in the population as a whole, following standardized procedures.Item Dislipidemia familiar e fatores associados a alterações no perfil lipídico em crianças(Ciência & Saúde Coletiva, 2019-01) Souza, Natália Aparecida de; Vieira, Sarah Aparecida; Fonsêca, Poliana Cristina de Almeida; Andreoli, Cristiana Santos; Priore, Silvia Eloiza; Franceschini, Sylvia do Carmo de CastroO objetivo deste artigo é avaliar a prevalência de dislipidemia e os fatores associados em crianças de 4 a 7 anos de idade. Estudo transversal, realizado com 402 crianças de 4 a 7 anos de idade, acompanhadas por um Programa de Apoio a Lactação nos primeiros seis meses de vida. Foram dosados colesterol total, triglicerídeos e lipoproteínas de alta (HDL) e baixa densidades (LDL). Selecionou-se variáveis que poderiam estar associadas ao perfil lipídico, como o histórico familiar de dislipidemia, estado nutricional e o consumo alimentar das crianças. Realizou-se análise de Regressão de Poisson com variância robusta. O nível de significância adotado foi de p < 0,05. Observou-se valores aumentados da LDL em 46,8% (188), colesterol total em 37,6% (151), triglicerídeos em 10,4% (42) e HDL abaixo do desejável em 33,8% (136) das crianças. Houve associação estatisticamente significativa entre o histórico familiar de dislipidemia com colesterol total, LDL e triglicerídeos (p < 0,05 e p < 0,001, respectivamente); Desmame precoce com LDL (p < 0,05); Sedentarismo com LDL e triglicerídeos (p < 0,05 e p < 0,001, respectivamente); bem como HDL com o consumo de bala (p < 0,05). Houve importante prevalência de alterações no perfil lipídico das crianças. São necessárias atividades de educação nutricional e programas voltados para esse grupo.Item Duration of breastfeeding in preterm infants followed at a secondary referral service(Revista Paulista de Pediatria, 2016-03-05) Freitas, Brunnella Alcantara Chagas de; Lima, Luciana Moreira; Priore, Silvia Eloiza; Franceschini, Sylvia do Carmo Castro; Carlos, Carla Fernanda Lisboa ValenteIdentify and analyze variables associated with shorter duration of breastfeeding in preterm infants. Retrospective cohort of premature infants followed up at secondary referral service in the period of 2010–2015. Inclusion: first appointment in the first month of corrected age and have undergone three or more consultations. Exclusion: diseases that impaired oral feeding. Outcome: duration of breastfeeding. A total of 103 preterm infants were evaluated, accounting for 28.8% of the preterm infants born in the municipality in that period, with a power of study of 80%. Descriptive analysis, t-test, chi-square test, Kaplan–Meier curves and Cox regression were used. p-values <0.05 were considered significant.The median duration of breastfeeding among preterm infants was 5.0 months. The risk of breastfeeding discontinuation among preterm infants with gestational age <32 weeks was 2.6-fold higher than for those born at 32 weeks or more and the risk of breastfeeding interruption in preterm infants who were receiving breastfeeding supplementation in the first outpatient visit was 3-fold higher when compared to those who were exclusively breastfed in the first consultation.The median duration of breastfeeding in preterm infants was below the recommended one and discontinuation was associated with gestational <32 weeks and the fact that the infant was no longer receiving exclusive breastfeeding in the first outpatient visit. When these two variables were associated, their negative effect on the median duration of breastfeeding was potentiated.Item Epidemiological characteristics and deaths of premature infants in a referral hospital for high-risk pregnancies(Revista Brasileira de Terapia Intensiva, 2012-10-01) Freitas, Brunnella Alcantara Chagas de; Sant'Ana, Luciana Ferreira da Rocha; Longo, Giana Zarbato; Siqueira-Batista, Rodrigo; Priore, Silvia Eloiza; Franceschini, Sylvia do Carmo CastroTo analyze the process of care provided to premature infants in a neonatal intensive care unit and the factors associated with their mortality. Cross-sectional retrospective study of premature infants in an intensive care unit between 2008 and 2010. The characteristics of the mothers and premature infants were described, and a bivariate analysis was performed on the following characteristics: the study period and the "death" outcome (hospital, neonatal and early) using Pearson's chi-square test, Fisher's exact test or a chi-square test for linear trends. Bivariate and multivariable logistic regression analyses were performed using a stepwise backward logistic regression method between the variables with p<0.20 and the "death" outcome. A p value <0.05 was considered to be significant. In total, 293 preterm infants were studied. Increased access to complementary tests (transfontanellar ultrasound and Doppler echocardiogram) and breastfeeding rates were indicators of improving care. Mortality was concentrated in the neonatal period, especially in the early neonatal period, and was associated with extreme prematurity, small size for gestational age and an Apgar score <7 at 5 minutes after birth. The late-onset sepsis was also associated with a greater chance of neonatal death, and antenatal corticosteroids were protective against neonatal and early deaths. Although these results are comparable to previous findings regarding mortality among premature infants in Brazil, the study emphasizes the need to implement strategies that promote breastfeeding and reduce neonatal mortality and its early component.
