Efeito da utilização da ginástica hipopressiva em mulheres com fibromialgia

Resumo
Introdução: A Fibromialgia (FM) é uma das doenças reumatológicas mais comuns e, nas últimas décadas, houve um incremento na produção de pesquisas que buscam tanto compreender melhor a sua fisiopatologia, ainda desconhecida, como melhorar o tratamento. Já é um consenso entre os pesquisadores da área da saúde que o exercício físico é fundamental para a melhora dos sintomas nos pacientes dignósticados com FM. No entanto, ainda há controvérsias sobre qual o melhor exercício, qual a intensidade, qual duração e quais potenciais riscos de levar à fadiga e gerar cinesiofobia. Objetivo: Analisar os possíveis efeitos da utilização da ginástica hipopressiva (GH), bem como o seu impacto na qualidade de vida de mulheres com fibromialgia da cidade de Viçosa/MG. Método: Foi utilizado um protocolo de exercícios de GH, ainda não testado em indivíduos com FM. A amostra foi composta de 10 mulheres com diagnóstico de fibromialgia, assim distribuídas: Grupo experimental (GEX) composto por 5 voluntárias com idade média de 37 anos e Grupo controle (GCT) também contendo 5 voluntárias, com idade média de 51 anos. A intervenção foi realizada de forma on- line, via ligação pelo aplicativo do WhatsApp. O tempo médio de duração das aulas de GH foi de 30 minutos realizadas duas vezes por semana, durante dois meses. Foram utilizados os seguintes instrumentos de avaliação para coleta de dados: Escala Visual Analógica – EVA para avaliar o nível de dor; FIQ (Fibromyalgia Impact Questionnaire) para análise do impacto gerado pela doença na qualidade de vida das mulheres; Questionário de Sensibilização Central (Brazilian Portuguese Central Sensitization Inventory – BP–CSI), para identificar se a predominância da característica da dor era de sensibilização central. Resultados: Todas as análises estatísticas foram realizadas pelo programa SPSS para Windows, versão 21.0 e para todos os tratamentos adotou-se um nível de significância de p<0,05. As voluntárias, após a participação no programa de GH, apresentaram: diminuição dos sintomas da FM; melhora na capacidade de andar vários quarteirões; aumento do número de dias da semana em que sentiram bem; diminuição da quantidade de dias da semana que deixaram de realizar atividades laborais; diminuição do cansaço ao se levantar; diminuição da sensação de rigidez corporal; diminuição dos sintomas de ansiedade e depressão; diminuição da sensação de dor. Conclusão: O programa de intervenção com exercícios do método da GH foi promissor para a melhora da qualidade de vida e na diminuição do nível de dor das mulheres pertencentes ao grupo de FM da cidade de Viçosa-MG. Introductions: Fibromyalgia (FM) is one of the most common rheumatologic diseases and, in the last decades, there has been an increase in the production of research aiming to better understand its pathophysiology, still unknown, and the best treatment. It’s common knowledge among health researchers that physical exercise is essential for improving symptoms in those patients. However, there are still some controversies on what is the best exercise, what is the intensity, duration, and potential risks of leading to fatigue and causing kinesiophobia. Objective: Analyze the possible effects of using hypopressive gymnastics (GH), as well as its impact on the quality of life of women with fibromyalgia in the city of Viçosa-MG. Methodology: a protocol of GH has been used, which hadn’t been tested in patients with FM. The sample group consisted of 10 women with a fibromyalgia diagnosis distributed as follows: Experimental Group (GEX) consisted of 5 volunteers with an average age of 37 years old and a Control Group (GCT), also consisted of 5 women, with an average age of 51 years old. The intervention was made online through WhatsApp video calls. The average duration of the GH classes was 30 minutes occurring twice a week for two months. The assessment instruments used for data collection were the following: Visual Analog Scale – VAS to assess the level of pain; FIQ (Fibromyalgia Impact Questionnaire) to analyze the impact generated by the disease on the women's quality of life; The Central Sensitization Inventory (BP–CSI) to identify whether the predominance of the pain characteristic was central sensitization. Results: All the statistical analyze were conducted by the SPSS program for Windows, version 21.0, and for all treatments a significance level of p<0.05 was adopted. After participating in the GH program, the volunteers obtained a decrease in FM symptomes regarding the capacity of walking many blocks; an increase in the number of days of the week in which they felt well, a decrease in the number of days of the week they had to stop performing work activities; a decrease in tiredness when getting up; decreased sensation of body rigidity; a decrease in anxiety and depression symptoms; decreased sensation of pain. Conclusion: the program of intervention with exercises using the GH method was promising in the improvement of quality of life and in the decrease of pain level in women belonging to the FM group in the city of Viçosa-MG.Keywords: Fibromyalgia.
