Utilização dos critérios de alta suspeição nos encaminhamentos oncológicos para um serviço terciário na microrregião da Zona da Mata mineira

Abstract
O câncer constitui um importante problema de saúde pública no Brasil, demandando estratégias que assegurem o diagnóstico precoce e o acesso oportuno ao tratamento no âmbito do Sistema Único de Saúde (SUS). Nesse contexto, os critérios de alta suspeição oncológica desempenham papel fundamental na organização dos fluxos assistenciais e na regulação do acesso aos serviços de alta complexidade. O presente estudo teve como objetivo analisar a utilização desses critérios na análise dos encaminhamentos de pacientes para um serviço terciário de oncologia na microrregião de saúde de Muriaé, Minas Gerais. Trata-se de um estudo observacional, retrospectivo, de abordagem quantitativa, com delineamento transversal, baseado em dados secundários extraídos do software AGESUS, referentes às solicitações de primeiro atendimento oncológico registradas no ano de 2023. Foram incluídos encaminhamentos não aceitos pelo Centro de Alta Complexidade em Oncologia (CACON) de referência e aqueles cujo primeiro atendimento ocorreu em prazo igual ou superior a 60 dias da solicitação. A análise dos dados foi realizada por meio de estatística descritiva, com cálculo de frequências absolutas e relativas, bem como medidas de tendência central, considerando o protocolo estadual de alta suspeição e a legislação vigente. Dos 1.165 encaminhamentos avaliados, 213 atenderam aos critérios de inclusão, dos quais 59 (51,18%) foram negados e 104 (48,82%) apresentaram tempo de espera igual ou superior a 60 dias. As negativas ocorreram predominantemente por ausência de elementos clínicos ou exames compatíveis com alta suspeição, sendo mais frequentes entre os encaminhamentos oriundos da Atenção Primária à Saúde. Observou-se, ainda, atraso expressivo na realização de biópsias. Conclui-se que há fragilidades na aplicação dos critérios de alta suspeição e no processo regulatório, evidenciando a necessidade de qualificação dos profissionais de saúde e fortalecimento da Atenção Primária, a fim de promover o diagnóstico precoce e a integralidade do cuidado ao paciente oncológico. In this context, criteria for high suspicion of cancer play a fundamental role in organizing care flows and regulating access to high-complexity services. This study aimed to analyze the use of these criteria in the analysis of patient referrals to a tertiary oncology service in the microregion of Muriaé, Minas Gerais. This is an observational, retrospective, quantitative, cross-sectional study based on secondary data extracted from the AGESUS software, referring to requests for initial oncological care registered in 2023. Referrals not accepted by the reference High Complexity Oncology Center (CACON) and those whose first care occurred within 60 days or more of the request were included. Data analysis was performed using descriptive statistics, calculating absolute and relative frequencies, as well as measures of central tendency, considering the state protocol for high suspicion and current legislation. Of the 1,165 referrals evaluated, 213 met the inclusion criteria, of which 59 (51.18%) were denied and 104 (48.82%) had a waiting time equal to or greater than 60 days. Denials occurred predominantly due to the absence of clinical elements or tests compatible with high suspicion, being more frequent among referrals originating from Primary Health Care. A significant delay in performing biopsies was also observed. It is concluded that there are weaknesses in the application of the high suspicion criteria and in the regulatory process, highlighting the need for qualification of health professionals and strengthening of Primary Care, in order to promote early diagnosis and comprehensive care for cancer patients.
