Título: Escala de avaliação do odor de feridas tumorais: construção e validação

Abstract: O odor das feridas tumorais é um sintoma de natureza subjetiva e por isso sua avaliação objetiva representa um grande desafio para a enfermagem. Na literatura, estão disponíveis instrumentos para sua avaliação, porém os mesmos não são validados. O objetivo geral deste trabalho foi desenvolver e validar uma escala de avaliação do odor de feridas tumorais. Os objetivos específicos foram: identificar na literatura os fatores relacionados à etiologia do odor das feridas tumorais; construir uma escala de avaliação do odor de feridas tumorais; verificar a validade de face e de conteúdo da escala proposta por especialistas da área de enfermagem em estomaterapia e cuidados paliativos. Trata-se de um estudo realizado em duas etapas, a saber: revisão integrativa da literatura e estudo metodológico. Na revisão integrativa foram identificados sete artigos publicados na língua inglesa, entre os anos de 2009 a 2018, que abordavam evidências sobre os fatores relacionados ao odor das feridas tumorais. Foram identificados três fatores principais associados ao odor das feridas tumorais: microorganismos aeróbios e anaeróbios, exsudato e tecido desvitalizado. Na segunda etapa, construiu-se a Escala de Avaliação do Odor de Feridas Tumorais com base nos fatores relacionados ao odor identificados na revisão integrativa. Posteriormente, foi feita a validade de face e conteúdo da escala por juízes convidados (n=17), selecionados por meio da Plataforma Lattes. Calculou-se o Índice de Validade de Conteúdo (IVC) para cada item e do instrumento. 58,3 % dos itens da escala obtiveram IVC de 0,94; 33,3 % dos itens obtiveram IVC de 0,88 e 8,3 % dos itens obtiveram IVC de 0,76. O IVC da escala foi de 0,91. Foi sugerido pelos juízes incluir o item exsudato sanguinolento, excluir o item avaliação bacteriana e incluir a avaliação do odor pelo paciente. Espera-se que a escala proposta possa auxiliar os profissionais de enfermagem no processo de mensuração do odor e colaborar com o raciocínio clínico no manejo do odor
The odor of tumor wounds is one of the most complex symptoms to approach and causes great suffering to the patient because it represents social isolation, depression, shame and embarrassment, impairing their quality of life. However, it is found in the literature that the instruments available for odor evaluation measure this symptom subjectively. The overall objective of this study was to validate an evaluation scale for odor of tumor wounds. The specific objectives were: to identify in the literature the factors related to otitis etiology of tumor wounds; construct a scale for evaluating the odor of tumor wounds; to verify the validity of face and content of the scale proposed by nursing specialists in stomatherapy and palliative care. It is a study was carried out in two stages, namely: integrative literature review and methodological study. In the integrative review, seven articles published in the English language between the years 2009 and 2018 were identified, which addressed evidence on factors related to the odor of tumor wounds. Three main factors associated with odor of tumor wounds were identified: aerobic and anaerobic microorganisms, exudate and devitalized tissue. In the second step, the Tumor Wound Odor Assessment Scale was constructed based on the factors related to the odor identified in the integrative review. Subsequently, the face and content of the scale were made by invited judges (n = 17), selected through the Lattes Platform. The Content Validity Index (CVI) was calculated for each item and instrument. 58.3% of the items on the scale obtained IVC of 0.94; 33.3% of the items obtained IVC of 0.88 and 8.3% of the items obtained IVC of 0.76. The IVC of the scale was 0.91. It was suggested by the judges to include the item bloody exudate, exclude the item bacterial evaluation and include evaluation of the odor by the patient. The scale proposed in this study may contribute to clinical reasoning in odor management.
