Título: Relação entre o nível de funcionalidade de pessoas com síndrome de Down e autismo e a qualidade de vida dos cuidadores primários
Relationship between the level of functionality of people with Down syndrome and autism and the quality of life of primary caregivers

Abstract: A chegada de uma pessoa com deficiência no âmbito familiar repercute em mudanças na rotina de todos os seus integrantes. Todavia um membro em específico assume as principais responsabilidades diante do sujeito com deficiência, sendo ele definido como o cuidador primário. O responsável pela pessoa com deficiência desempenha atividades voltadas para o cuidado de forma rotineira e ininterrupta, e em alguns casos o impacto das ações exercidas pode resultar em prejuízos na Qualidade de Vida (QV) do cuidador, que chega a negligenciar o autocuidado para se dedicar exclusivamente à pessoa com deficiência. Para além das tarefas do cuidado cotidiano executado pelo responsável, outros fatores podem influenciar o seu bem- estar, como: a funcionalidade do sujeito sob seu cuidado, a idade da pessoa com deficiência e a do cuidador, o nível socioeconômico (NSE) da família, os dados sociodemográficos, entre outros aspectos, que podem ou não potencializar ou prejudicar a QV do responsável. A partir dos questionamentos supracitados, este trabalho teve como objetivo mensurar as Atividades de Vida Diária (AVDs) de pessoas com Transtorno do Espectro Autista (TEA) e sujeitos com síndrome de Down (SD), além de correlacioná-las com a QV e com o nível socioeconômico de suas cuidadoras primárias. Trata-se de um estudo transversal, realizado com 27 cuidadoras primárias de pessoas com TEA e 31 cuidadoras de indivíduos com SD de 47,38 ± 13,71 anos residentes nos municípios de Ponte Nova, Rio Branco, Teixeiras, Ubá e Viçosa, no interior da Zona da Mata de Minas Gerais/Brasil. Os instrumentos empregados na coleta de dados foram a Escala de Atividade de Vida Diária de Waisman (EAVD-W), para avaliar o nível de funcionalidade dos sujeitos com SD e TEA; e o World Health Organization Quality of Life versão abreviada e em Português (WHOQOL-Bref), para estimar a QV das cuidadoras. Com a finalidade de caracterizar a amostra, as participantes responderam ao Critério de Classificação Econômica da Associação Brasileira de Empresas de Pesquisa e a um Questionário Sociodemográfico, este último elaborado pela pesquisadora. Na análise dos dados, inicialmente o teste de Komolgorov-Smirnov foi empregado com o intuito de verificar a normalidade dos dados coletados, com posterior exploração descritiva das variáveis estudadas. Utilizaram-se o teste t Student para comparação das médias entre os grupos independentes com distribuição normal e o teste Mann-Whitney para análise dos dados não paramétricos. O teste de Kruskal-Wallis com post hoc de Dunn’s foi aplicado para comparação das AVDs com as faixas etárias (criança, adolescente e adulto) das pessoas com deficiência. Além disso, foi aplicada a correlação de Spearman para efetivar três análises, em que a primeira corresponde às variáveis pertencentes às cuidadoras de pessoas com SD e TEA, que são: QV e NSE. As outras duas análises realizadas são inerentes aos grupos de cuidadoras e aos grupos de pessoas com deficiência; trata-se das seguintes variáveis: AVDs e NSE e AVDs e QV, Em todos os tratamentos, adotou-se um nível de significância de 5%. Os principais resultados apontaram que os sujeitos com SD 14,00 (3-25) são mais independentes que os indivíduos com TEA 10,00 (0-21). Observou-se diferença significativa da aquisição das AVDs durante o desenvolvimento dos sujeitos com deficiência. No que diz respeito à comparação entre as variáveis (QV, idade e o NSE) das cuidadoras, foi encontrada diferença significativa nas relações sociais SD = 75,00 (0-100), TEA = 58,33 (17-100), na idade SD = 53 (24-84), TEA = 39 (23-55) e no nível socioeconômico SD = 18 (10-45) e TEA = 25 (12-56). Já no que tange às correlações realizadas, foram encontrados resultados significativos entre as variáveis QV e NSE das cuidadoras, no domínio Meio Ambiente do Grupo Geral (r = 0,441) e do Grupo SD (r = 0,394). Também foi identificada a correção significativa entre as variáveis AVDs e QV no domínio Físico (r = 0,268) do Grupo Geral (SD + TEA) e nos domínios Físico (r = 0,389) e Meio Ambiente (r = 0,390) do Grupo SD. Conclui- se que os indivíduos com SD têm mais habilidades de vida diária do que as pessoas com TEA, resultado que deve ser analisado com cautela, pois já existe diferença nas faixas etárias entre os grupos. Já no tocante ao perfil das cuidadoras, notou-se que as responsáveis por pessoas com autismo demonstraram prejuízo em sua QV, especificamente no domínio Relações Sociais, em comparação com as cuidadoras de sujeitos com SD. Ainda sobre a caracterização das responsáveis, pôde-se observar que o Grupo SD demonstrou ter a idade mais avançada e pertencer a um nível socioeconômico mais baixo em relação ao Grupo TEA. Por fim, pode-se compreender que o aumento do NSE do Grupo SD tem relação direta com a melhoria da QV de suas cuidadoras, especialmente no que se refere ao domínio Meio Ambiente. Além disso, a aquisição das AVDs de pessoas com SD influi, de forma positiva, nos domínios Físico e Meio Ambiente de suas cuidadoras.
The arrival of a disabled person in the family repercussions changes in the routine of all its members, however, a specific member assumes the main responsibilities towards the subject with disability, being defined as the primary caregiver. The person responsible for the person with the disability performs routine and uninterrupted care activities, in some cases the impact of the actions taken may result in impairment of the caregiver's quality of life (QoL), which neglects self-care to devote exclusively the disabled person. In addition to the day-to-day care tasks performed by the caretaker, other factors may influence his or her well-being, such as the functionality of the subject under his or her care, the age of the disabled person and the caregiver, the family's socioeconomic status , socio-demographic data, among other aspects, that may or may not enhance or impair the QoL of the person in charge. Based on the aforementioned questions, this study aimed to measure the daily life activities (ADLs) of people with Autism Spectrum Disorder (ASD) and subjects with Down syndrome (SD), in addition to correlating them with QOL and socioeconomic level of their primary caregivers. This was a cross-sectional study, carried out with 27 primary caregivers of people with ASD and 31 caregivers of individuals with SD of 47.38 ± 13.71 years living in the municipalities (Ponte Nova, Rio Branco, Teixeiras, Ubá and Viçosa). of the interior of the Zona da Mata of Minas Gerais / Brazil. The instruments used to collect data were the Waisman Daily Life Activity Scale (EAVD-W) to assess the functional level of subjects with SD and TEA, the World Health Organization Quality of Life (WHOQOL- Bref) to estimate the QoL of the caregivers, and, in order to characterize the sample, the participants answered two questionnaires, namely: the Economic Classification Criteria of the Brazilian Association of Research Companies and a sociodemographic questionnaire, the latter elaborated by the researcher . For the analysis of the data, initially the Komolgorov-Smirnov test was used with the intention of verify the normality of the collected data, with subsequent descriptive exploration of the studied variables. The Student's t-test was used to compare the means between the independent groups with normal distribution and the Mann-Whitney test for the non-parametric data. Dunn's post hoc Kruskal-Wallis test was applied to compare ADLs with the age groups (children, adolescents and adults) of people with disabilities. In addition, the Spearman correlation was applied to perform three analyzes, the first correlation corresponding to the variables belonging to the caregivers of people with SD and TEA, which are: QV and NSE. The other two correlations are inherent to the groups of caregivers and groups of people with disabilities. These are the following variables: AVDs and NSE and AVDs and QV. For all treatments a significance level of 5% was adopted. The main results indicate that subjects with SD are more independent 14.00 (3-25) than individuals with ASD 10.00 (0-21). There is a significant difference in the acquisition of ADLs during the development of subjects with disabilities (SD and TEA), strictly between the stages of childhood 10 (0-18) and adult 16 (5-25). Regarding the comparison between the variables (QoL, age and NSE) of caregivers of people with DS and TEA, a significant difference was found in Social Relations SD = 75.00 (0-100); TEA = 58.33 (17-100), at age SD = 53 (24-84); TEA = 39 (23-55) and at the socioeconomic level SD = 18 (10-45); TEA = 25 (12- 56). Regarding the correlations, significant results were found among the variables, QoL and NSE of the caregivers, in the environmental domain of the general group (r = 0,441) and in the SD group (r = 0.394). It was also identified a significant correlation between the AVDs and QV variables in the physical group (r = 0.268) of the general group (SD + TEA) and in the physical (r = 0.389) and environment (r = 0,390) domains of the SD group. It is concluded that individuals with DS have more daily life skills than people with ASD, this result should be analyzed with caution, since there is a difference of the age groups between the groups. Regarding the profile of caregivers, it was noted that those responsible for people with autism demonstrated impairment in their QOL, specifically in the social relations domain when compared to caregivers of subjects with DS. Still on the characterization of the responsible ones, it can be observed that the SD group showed to be the most advanced age and to belong to a lower socioeconomic level in relation to the TEA group. Finally, it can be understood that the increase in the NSE of the SD group is directly related to the improvement of the QoL of their caregivers, especially in relation to the environment domain. In addition, the acquisition of ADLs of people with DS has a positive influence on the physical and environmental domains of their caregivers.
